Content: The mechanism of ejaculation and orgasm have been studied for decades through basic research, but this knowledge can sometimes seem quite complex to a nonphysician. Francesca and Evie hosted one of the world’s top scientists committed to studying the biological mechanisms involved in male ejaculation and orgasm. This interview aims to produce distilled information from these decades of basic research and translate this knowledge into management tips for clinical practice. Several issues have been touched on: whether ejaculation and orgasm are two different entities, which biological mechanisms are involved, how psychological (emotional and behavioral) factors affect ejaculation and the ability to orgasm, and which is the pathway. Premature and delayed ejaculation has been discussed to clarify if behavioral predispositions can explain these conditions, what basic research has shown about the impact of frequent masturbation and porn, and the role of performance anxiety in erectile dysfunction and ejaculation disorders. Important information on painful ejaculation has been highlighted too. What we sexologists need to first rule out before assuming psychological factors and the most common mistakes we fall into when treating men with ejaculatory disorders were examined. These and other matters have been answered by Prof Giuliano, whose comments were evidence-based and with clear clinical implications.

François Giuliano, MD-PhD, is a Professor of Therapeutics at the University of Versailles-Saint Quentin and has a specialist urological practice at the R. Poincaré Academic Hospital of Garches, France. His practice focuses on sexual medicine, especially in the context of neurological diseases.

Prof Giuliano’s major research interests include the pathophysiology, diagnosis, and management of sexual dysfunction, including erectile dysfunction and ejaculatory disorders.
He has extensive experience in conducting clinical trials. He is also interested in an evidence-based approach to clinical practice and has participated in developing clinical epidemiology and medical education. He has an active research laboratory investigating the pharmacological mechanisms controlling ejaculation, erectile function, and female sexual and lower urinary tract functions, with a particular interest in neural control involving an integrated approach using neuroanatomical, neurophysiological, biochemical, and pharmacological techniques.
Francois Giuliano is a widely published author in peer-reviewed journals, has edited and contributed to several books, and is involved in producing guidelines and care pathways.
He was Vice-Chairman of the International Consultation of Sexual Dysfunctions in 1999, 2004, 2009, and 2015 and has won several national and international awards. He is the past president of the European Society of Sexual Medicine.
